Application submission using SEKAP and Planning Portal

This tool allows for generating administrative applications e.g. concerning making or changing MPZP records or SUiKZP. The tool actuates after clicking the icon and choosing the “Applications registration” tool. Then, the tool window with “Form selection” is triggered. The following steps present how to work with the tool.

Step I – indication of the area, to which the application applies. You can do this in 4 different ways;

1. Via area and plot number selection.

This option contains two drop-down lists – first enter the area number (a hint will appear) or select it directly from a drop-down list. After selecting the area, another list is activated – a plot number. In this case it is also possible to enter it directly or to select it from the list, where numbers of plots can be found in a given area.

2. Via address selection.

This option also contains two drop-down lists – first enter the street name (a hint will appear) or select it directly from the drop-down list. After selecting the street, another list is activated – a building number. In this case it is also possible to enter it directly or to select it from the list, where you can find numbers of buildings on a given street.

3. Via plot indication (one or many).

On the map from the plot vector layer. After checking the “Indicate lot/lots on the map” option and clicking “Indicate” button, a point selection section will activate. By clicking with left mouse button, you select one plot on the map. After selecting the plot, a window with information about plot number and area to which it belongs is triggered. Operation can be repeated in such a way that a user can select all the plots they are interested in. After finishing plots preview, click OK.

4. Via drawing on the area map.

After checking this option and clicking the “Draw” button, the drawing option is actuated. It allows to draw one area on the map in any shape and size.

Step II – selection of the form of application submission

Two options are available:

1. traditional – – printed form submitted to the Municipal Office. Upon selecting this form, you go to further tabs where the applicant’s personal data and detailed information on the application are to be entered. When all required fields are completed, the application is generated. Download, print, sign and submit it to the Municipal Office via electronic mail or by traditional post service.

2. electronic – e-PUAP.This form of application generating shall not require to fill out additional fields, besides the „Form selection” tab, since the data shall be given in the e-PUAP platform and the application generated by the said tool shall be an electronic attachment presenting the application location.

Step III – form selection

From the drop-down list select a form from forms available for a given area, e.g. for the issue of discharge and map extracts from the Study – SUiKZP etc. (Fig. 3). The form selection activates the “Next” button which allows to move to the “Personal data” tab.

Step IV – filling in the personal data

You need to select applicant (natural or legal person) and then fill in necessary boxes. In the case of natural persons these are as follows: name, surname, PESEL, e-mail, phone number; legal persons: name, company’s headquarters, National Business Registry Number, tax number, e-mail and phone number. In both cases it is necessary to fill in the applicant address.

Red frames mean that you have not filled obligatory boxes or that information is incorrect (too short/long numbers, incomplete e-mail address). If the mailing address is different from the residential/permanent/headquarters address, it is necessary to check the box. Check if you want to give a mailing address. Tool window will be extended by additional boxes.

Step V – filling detailed data

The last tab is for indicating the subject and the scope of the requested change, contents of the proposed comment together with justification; it is possible to write some additional, useful information. You also need to select form of answer reception from the Municipal Office and verify the correctness of the entered data.

After clicking the “Download” button – in the lower right-hand corner of the window, the Google security reCAPTCHA will be actuated (securing from automated applications submission by so-called internet bots), and then the document will be downloaded.
