    GENERAL INFORMATION ON PROTECTION OF PERSONAL DATA COLLECTED BY THE MUNICIPAL OFFICE IN GLIWICE1

    1. The personal data controller is the Mayor of the City of Gliwice with its registered office at the Municipal Office in Gliwice (ul. Zwycięstwa 21, 44-100 Gliwice).

    2. We collect and process personal data for the purpose of:

    • performance of obligations and rights resulting from the provisions of law in connection with the performance of own public tasks commissioned or performed on the basis of agreements referred to, in particular, in Article 7 and 8 of the Act on Municipal Self-Government and Article 4 of the Act on County Self-Government; failure to provide data will result in consequences provided for in the provisions of law;
    • concluding and executing civil law and labour law contracts; failure to provide data will make it impossible to conclude or execute a contract.

    If it is not expressly required by law to provide personal data, you may be asked to give your consent to the processing of personal data for a specific purpose and scope. In this case you always have the right not to consent. If you do not give consent, your data may not be used for the purpose for which your consent was requested.

    3. Any data subject may exercise the following rights:

    • request access to the content of his/her personal data, rectify (correct) personal data, delete or restrict the data processing;
    • object to the processing of his/her personal data;
    • in the case of data collected with consent, you may also withdraw the consent; the consent withdrawal shall not affect the data processing until the consent has been withdrawn.

    In exceptional cases, where permitted by law, a data subject may request that data processed by automated means be transferred to another data controller. However, this does not apply to data processed in the public interest or in the exercise of official authority. The rights may be exercised by submitting a traditional form of documents with a signature in the Municipal Office in Gliwice or by sending electronic correspondence using a general letter on the SEKAP/ePUAP platform, confirmed by a Trusted Profile or a qualified electronic signature. Special provisions may introduce additional requirements related to the exercise of the aforementioned rights, in which case you will be informed thereof.

    Any data subject shall also have the right to lodge a complaint against the processing of data contrary to the law with the supervisory authority, which is the President of the Office for Personal Data Protection.

    4. If you have any questions or concerns regarding the processing of your personal data collected by the Authority, you may contact the Data Protection Officer via e-mail iod@um.gliwice.pl. Persons who do not use electronic mail may submit their enquiries in writing, by telephone or in person to the minutes at the Main Information in the hall of the Municipal Office ul. Zwycięstwa 21 (tel. 239-11-65 or 239-12-54) indicating the form they expect a reply and providing the contact details necessary to respond.

    5. Additional information:

    a) personal data may be transferred:

    • to other entities, including municipal organisational units to perform public tasks,
    • to other persons or entities, if the law imposes an obligation on the City Mayor to make the processed data available or to make it publicly available

    b) the data are kept for the period resulting from the Prime Minister’s Regulation on the office instruction, uniform material file lists and instructions on the organisation and scope of operation of company archives, or special provisions,

    c) there is no automated decision making in individual cases at the Office, including profiling.

    Personal data safety

    The personal data controller - the Mayor of the City of Gliwice informs that the persons whose personal data are processed by the Municipal Office in Gliwice may contact the Data Protection Officer in all matters related to the processing of their personal data and the exercise of their rights under Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016.
